For decades firefighters have warned that the hazards of their work extend far beyond the blaze itself. Toxicants such as benzene released from burning plastics, formaldehyde generated by smoldering wood and paper, and asbestos present in older structures are absorbed through skin, inhaled, or retained in protective gear.
Over time, these exposures have been linked to higher incidences of cancers—including mesothelioma and leukemia—among fire‑service personnel. On April 15, 2023, the U.S. National Institute for Occupational Safety and Health (NIOSH) opened the National Firefighter Registry for Cancer, an online questionnaire designed to collect uniform data from firefighters nationwide. The registry, created under the Firefighter Cancer Registry Act of 2018, seeks to fill the long‑standing gap of a coordinated, national picture of cancer rates within the profession.
Prior to the registry, cancer tracking was fragmented. Individual fire departments kept their own records, and a few states attempted separate counts, but no single system aggregated the information.
This lack of comprehensive data prevented researchers from pinpointing which occupational exposures posed the greatest risk and limited the ability to develop targeted prevention strategies. The new questionnaire is voluntary and web‑based. It asks participants to detail their occupational history, length of service, types of fires fought, and specific exposures encountered on the job, as well as lifestyle and health information. Each response adds to a growing dataset that NIOSH will analyze to map cancer risk across the fire service.
NIOSH brings extensive experience in occupational health research, having studied workplace hazards for decades. The agency is equipped to design the registry, safeguard participant privacy, and translate raw data into actionable recommendations.
While the registry is not a treatment or vaccine, it is intended as a research tool that can ultimately guide interventions such as improved protective equipment, standardized decontamination procedures after incidents, and refined medical‑screening guidelines for firefighters identified as high‑risk. Implementation of the registry marks a concrete response to the fire‑service community’s calls for action. Firefighters have witnessed colleagues succumb to cancer and have long sought systematic data to support prevention efforts.
