LOS ANGELES — The next move belongs to the researchers. After two years of giving 365 older adults high-dose fish oil supplements and watching them show no cognitive benefit, the team at Keck Medicine of USC is now asking a harder question: who might actually need these nutrients? The study just published is not the end of the road.
It is a fork in it. Lead investigator Hussein Naji Yassine, MD, director of the USC Center for Personalized Brain Health, said flatly that fish oil supplements do not appear to protect brain health.
His team confirmed the omega-3s reached the brain — the DHA showed up in cerebrospinal fluid. It just did nothing measurable for memory or cognition. That result closes one door.
But Yassine’s center is already working on opening another. The next phase, he has indicated, will focus on who among the study participants might have been helped if the trial had run longer or targeted a different subgroup.
The key variable is genetics. Nearly half the participants — 47% — carried the APOE4 gene, the strongest known genetic risk factor for late-onset Alzheimer’s. That is a high number.
It was deliberate. The study recruited people at elevated risk. But the gene itself may determine whether omega-3s ever work for anyone.
Earlier lab work from the same USC team suggested that APOE4 carriers process DHA differently. Their brains may not use the fatty acid the same way non-carriers do.
If that is true, then future trials cannot lump all older adults together. They will need to separate groups by genotype. That is the concrete next step: a trial designed to test omega-3s only in people without the APOE4 gene, or perhaps only in those with it.
The data to design that trial now exists. The question is funding.
The National Institutes of Health funded the current study. A follow-up would require a new grant application. No timeline has been announced.
But Yassine has publicly stated that personalized approaches are the way forward. The center he runs is built on that premise. For consumers, the decision is simpler.
The supplement industry sells fish oil as a brain booster. This trial — placebo-controlled, double-blinded, two years long — found no evidence for that claim in people who rarely ate fish.
The figure that matters: zero measurable benefit. For investors, the math is different. The global fish oil supplement market is worth billions.
A single rigorous trial showing no effect does not kill a product category. But it shifts the burden of proof.
Companies that market omega-3s for cognitive health now face a study that says the nutrients reach the brain and still do nothing. The next data point to watch is replication. Other labs will try to confirm or challenge the USC findings.
If they hold, the conversation moves from “does it work?” to “for whom does it work?” That question is harder. It requires bigger trials, longer timelines, and genetic screening. Yassine’s team has the patient data.
They have the cerebrospinal fluid samples. They have the cognitive scores.
What they do not have yet is a clear path to a targeted trial. That is the next decision to watch.
